Tweed Community Options Project

HACC Case Management Services

Tweed Shire Council is the auspice for Tweed HACC Case Management services. These services are individualised community resources and supports, utilising Grant Funding from NSW State Government’s Department of Ageing Disability and HomeCare (DADHC). There are other similar programs in operation throughout NSW utilising the same Guidelines and Grant Funding source.

The recurrent Annual Grant enables the Tweed Community Options Project to provide Case Management services to Tweed Shire residents who meet the Commonwealth/State Governments’ HACC eligibility criteria.

Podiatry Services

In addition to our case management program, Tweed Community Options have a three year grant to provide Podiatry Services for eligible residents of the Tweed Shire. Applicants must be eligible for HACC services. A Podiatry Brochure Sept 2008 (click here to download...) and referral form (click here to download...) are available to provide further information. You can also self refer by calling our office on (07) 5569 3110 and ask for our Information Package or ask to be on our wait list, should we be ‘at capacity’ when you call. This grant finishes on 30th June 2009 or earlier should the funding be exhausted. We have 100 eligible HACC clients in the program at the time of updating this website information sheet (30/9/2008).

Older Parent Carers

Tweed Community Options are also currently providing services to Older Parent Carers of people who have a disability. Priority is for “hidden” carers to receive respite support and we are actively seeking referrals into this program. It is a two year project funded by the Commonwealth and State Governments ($1.1 Million). Reports

In the 2007/2008 financial year Tweed Community Options provided a range of services directly to 845 individuals utilising grant funds from both NSW Health and NSW Department of Ageing Disability and HomeCare. Our most recent Annual Report ( click here to download...) is linked to this page along with our recent Questionnaire Reports that detail consumer feedback on our case management services. We believe that reading these reports will assist you to further understand the work undertaken at Tweed Community Options and the consumer outcomes we achieve.

Other HACC Services

There are other HACC services in the Tweed that other organisations administer, and a brochure (click here to download...) on HACC Tweed Services and their contact details is available.

Other funding provided for the Tweed, but within larger funding allocations for distribution across the Far North Coast are explained below.

The Regional Integrated Support Services (RISS) are a HACC funded service providing domestic and lawn-moving services. The Short-term Time out Project (STOP) is a disability funded flexible respite service. These regional funded projects are managed by Clarence Valley Council’s Community Services Programs (Ballina and Maclean Offices). Contact the Team Leader via email on sonia.lee@clarence.nsw.gov.au and request their Brochures and also the Application Forms for STOP and for RISS.

Other respite funding available across the Far North Coast is provided by a HACC grant and DADHC disability funding, through the Flexible Respite Options Consortium (FROC) and information and application forms are available from the Tweed Valley Respite Service. ComPacks

Currently Tweed Community Options are also providing ComPacks services (click here to download handbook...) to discharge patients of the Tweed Hospital. The Tweed Hospital is the only source of referrals to Tweed Community Options as this project is funded through NSW Health. More than 1000 residents in the Tweed have benefited from this project in recent years. Currently Tweed Community Options accepts referrals of up to 300 per year from the Tweed Hospital staff.

Specific Assistance for Carers

NSW Health had made available a grant of funds to support the establishment of a website specifically to help working carers. The site design and updating is provided by DAISI (Disability and Aged Information Service) who received a grant from NSW Health for this Project. Service Providers – Service Agreements

Tweed Shire Community Options Service Agreements are used for all Brokerage services and these agreements are required prior to commencing work on our behalf. All service providers with us, need to be aware that our Service Agreement states that OH&S legislation and practices; Home and Community Care (HACC) Guidelines and practices; and client/carer rights must be adhered to when delivering services to our clients. Service providers are required to read these documents and implement them when sub-contracting with Tweed Shire Council’s Community Options Project.

If you wish to commence or continue providing brokered services to Tweed Community Options clients, please forward TWO signed copies of the Service Agreement (click here to download...) to, Tweed Shire Community Options Project PO Box 651 TweedMall Post Shop, TWEED HEADS NSW 2485. Please ensure the checklist on Page 2 of the Schedule 1 is checked and all documents from Schedule 1 & 2 as well as Appendix 1 & 2 are duly authorised and returned.
